Gabapentin for Sciatica: Evidence, Limits and Safe Questions

Quick answer

Gabapentin for sciatica is a common prescription, yet UK guidance says not to offer gabapentinoids for sciatica because there is no overall evidence of benefit and there is evidence of harm (NICE NG59, Low back pain and sciatica). If you take it, do not stop on your own. Talk with your prescriber about whether it is working for you.

Guidance01

Is gabapentin recommended for sciatica?

NICE says not to offer gabapentinoids, other antiepileptics, oral corticosteroids or benzodiazepines for sciatica.

The NICE low back pain and sciatica guideline (NG59) reasons that there is no overall evidence of benefit and there is evidence of harm (NICE NG59, Low back pain and sciatica). That guideline is from the UK, and doctors elsewhere may follow other advice.

Gabapentin is often prescribed for nerve pain from other causes, including diabetic neuropathy (NIDDK, Peripheral Neuropathy). Sciatica is a different problem, so the same medicine may not fit.

The trial02

What does research say about gabapentin-type drugs for sciatica?

The best sciatica trial we found tested pregabalin, a close relative, and it did no better than placebo.

In a trial of 209 people with sciatica, pregabalin did not beat placebo on leg pain at week 8 (Mathieson et al., NEJM 2017 (PRECISE)). We found no matching trial of gabapentin itself in our library.

If you take it03

What should you do if you already take gabapentin?

Keep taking it as prescribed until your prescriber tells you otherwise.

Never stop or change it alone

Stopping a prescription on your own can cause problems. Bring your questions to the prescriber who wrote it.

  • Ask what the goal is and how you will judge whether it helps
  • Ask about drowsiness and dizziness, which were more common with pregabalin (Mathieson et al., NEJM 2017 (PRECISE))
  • Tell your pharmacist about any supplement you add

Other options04

What else do guidelines suggest for sciatica?

Staying active comes first: NICE recommends self-management, normal activity and considering exercise.

ACP also puts non-drug care first for back pain (Qaseem et al., ACP guideline 2017). Read our relief options guide for the full list.

FAQ05

Gabapentin and sciatica: questions

Why do doctors prescribe gabapentin for sciatica if guidelines advise against it?

Practice varies, and some doctors weigh nerve pain differently. The NICE guideline advises against it for sciatica (NICE NG59, Low back pain and sciatica). Ask your prescriber to explain the reasoning for your case.

Does gabapentin treat the cause of sciatica?

Gabapentin is not described as fixing a pressed nerve root. NIDDK says such medicines for neuropathy relieve pain but do not reverse nerve damage (NIDDK, Peripheral Neuropathy). Causes like a disk need their own assessment.

Can I stop gabapentin if it does not help?

Do not stop on your own. Ask your prescriber how to review it and, if needed, how to taper. They may also suggest other options from our relief guide.

Is gabapentin the same as pregabalin?

They are related medicines, often grouped as gabapentinoids (NICE NG59, Low back pain and sciatica). They are separate drugs with their own doses. Our pregabalin trial post covers the sciatica study.

Can SciatiEase replace gabapentin?

No. SciatiEase is a dietary supplement and has no sciatica trial of its own. Never swap a prescription for a supplement; talk with your prescriber first.
