Guide: sciatica basics

What Is Sciatica? Sciatic Nerve Pain, Causes, Symptoms and Red Flags

Quick answer

Sciatica is pain, tingling, numbness or weakness in the leg caused by pressure on or damage to the sciatic nerve, most often from a herniated disc. It is a symptom, not a disease of its own. Most cases improve over weeks, though about 20-30% of people still have problems one or two years later.

Definition01

What is sciatica?

Sciatica is a set of leg symptoms from pressure on, or damage to, the sciatic nerve (MedlinePlus, Sciatica).

The word names where the trouble travels, not what started it. Pain, weakness, numbness or tingling can reach the buttock, thigh and leg. Doctors treat it as a sign of another problem, such as a disc pressing on a nerve root.

Back pain ranks among the most common medical problems in the United States (NIAMS, Living With Back Pain). Sciatica is the branch of it that runs down the leg.

Causes02

What causes sciatic nerve pain?

A herniated disc is the usual cause. Spinal stenosis and piriformis syndrome are two others (MedlinePlus, Sciatica).

Each cause squeezes or irritates the nerve at a different spot. Most sciatica comes from a nerve root near the spine, where discs and bone sit close together (Koes et al., BMJ 2007).

You may also see the phrase "sciatic nerve starvation" in ads. It is not a medical term, and no source we checked uses it.

Spinal stenosisThe space around the nerves narrowsMedlinePlus (MedlinePlus, Sciatica)
Piriformis syndromeA buttock muscle irritates the nerveMedlinePlus (MedlinePlus, Sciatica)
Cauda equina syndromeThe bundle of roots at the base of the spine is squeezed; an emergencyAANS (AANS, Cauda Equina Syndrome)

Symptoms03

What are the symptoms of sciatica?

Pain, burning, tingling, numbness or weakness, usually down one leg.

Sciatica often eases by itself but commonly returns (MedlinePlus, Sciatica). The pattern helps tell it apart from other nerve problems.

  • Pain that travels from the buttock into the thigh or leg
  • Tingling, numbness or a weak leg on the same side
  • Flare-ups that settle, then come back months later

Burning or tingling in both feet, often worse at night, fits diabetic peripheral neuropathy better. It affects up to half of people with diabetes (NIDDK, Peripheral Neuropathy).

Red flags04

When is sciatica a red flag for cauda equina syndrome?

When leg pain comes with numbness in the saddle area, trouble passing urine, loss of bowel control or leg weakness (AANS, Cauda Equina Syndrome).

Trouble passing urine is the most common sign. Surgery within 48 hours of the first symptoms gives a clear advantage for recovery (AANS, Cauda Equina Syndrome).

Back pain with fever needs care right away as well (MedlinePlus, Sciatica), and so does back pain with unexplained weight loss (NIAMS, Living With Back Pain).

  1. Numb groin or inner thighsThe 'saddle' area you would sit on
  2. New bladder or bowel troubleCannot pass urine, or leaks you cannot control
  3. Leg weakness getting worseFoot drop, stumbling, or one leg giving way
  4. Pain after a fall or crashOr with fever, weight loss or a cancer history
  5. Pain in both legsEspecially with numbness spreading on both sides

Most casesMost sciatica improves over weeks with time, staying active and simple self-care. See your doctor if pain is severe, lasts more than a few weeks or keeps coming back.

Call 911Or go to the emergency room for saddle numbness, new bladder or bowel problems, or fast-worsening leg weakness. These can mean cauda equina syndrome, which needs care within hours.

Signs from AANS and NHS-style guidance (AANS, Cauda Equina Syndrome) and MedlinePlus (MedlinePlus, Sciatica). A supplement is never a reason to wait.

Time course05

How long does sciatica last?

Most acute sciatica settles over weeks, but about 20-30% of people still have problems after one or two years (Koes et al., BMJ 2007).

Doctors sort back pain by how long it has lasted. That label shapes which care comes next.

  1. Days to weeks

Acute back pain starts suddenly and usually lasts a few days to a few weeks (NIAMS, Living With Back Pain).

  2. 4 weeks

Call a provider if pain is still there (MedlinePlus, Sciatica).

  3. 6-8 weeks

The usual window for conservative care before other options (Koes et al., BMJ 2007).

  4. 12 weeks and beyond

Pain past 12 weeks counts as chronic (NIAMS, Living With Back Pain).

Self-care06

What can you do at home for sciatica?

Stay active, skip bed rest, and try ice first, then heat (MedlinePlus, Sciatica).

NICE advises self-management and keeping up normal activities (NICE NG59, Low back pain and sciatica). In a Cochrane review of 249 trials, exercise cut chronic low back pain by about 15 points on a 100-point scale, though sciatica was not studied on its own (Hayden et al., Cochrane 2021).

The dial shows one ordinary day. The midday walk is backed by better evidence than any capsule on it.

2 capsules with food and water, as the sales page suggests. Prescribed medicine stays on its own schedule.

  2. A walk at midday

Staying active is the first-line advice for sciatica in national guidelines; bed rest is not.

  3. Dinner, about 6:30

The other 2 capsules complete the 4-capsule serving on the panel. Never more than 4 a day.

  4. Night, 7 to 9 hours

Activity advice from NICE (NICE NG59, Low back pain and sciatica) and the American College of Physicians (Qaseem et al., ACP guideline 2017); herb notes from NCCIH (NCCIH, Passionflower). The capsule times are a suggestion; follow your label.

Supplements07

Where does a supplement like SciatiEase fit with sciatica?

At the edge. A capsule cannot move a disc, and no trial has tested SciatiEase in sciatica.

PEA, its largest ingredient at 500 mg, has one 3-week sciatica trial of 636 people at 300 or 600 mg a day (Keppel Hesselink, J Pain Res 2015). A full serving lands between those doses. See the ingredients for the rest of the label.

Keep taking prescribed medicine

Never drop or lower a prescribed pain or nerve medicine because you began a supplement. The prescriber decides changes.

Seeing a doctor08

When should you see a doctor about sciatic nerve pain?

When pain lasts past 4 weeks, keeps getting worse, or brings numbness, tingling or weakness (MedlinePlus, Sciatica) (NIAMS, Living With Back Pain).

Bring a short note: when the pain began, what eases it, and every medicine and supplement you take (NCCIH, Using Dietary Supplements Wisely).

FAQ09

Sciatica questions, answered plainly

Can sciatica go away on its own?

Often, yes. MedlinePlus says sciatica frequently improves without treatment (MedlinePlus, Sciatica), and most people with acute sciatica do well (Koes et al., BMJ 2007). Staying active helps. See a provider if it lasts past 4 weeks or any red flag appears.

Is sciatica a nerve problem or a muscle problem?

It is a nerve problem: the sciatic nerve or one of its roots is pressed or irritated. A muscle can be the cause, though. In piriformis syndrome, a buttock muscle irritates the nerve (MedlinePlus, Sciatica).

What is cauda equina syndrome?

It is a rare emergency in which the bundle of nerve roots at the base of the spine is squeezed. Signs include saddle numbness, trouble passing urine and leg weakness. Surgery within 48 hours of the first symptoms helps recovery most (AANS, Cauda Equina Syndrome).

Should I rest in bed with sciatica?

No. MedlinePlus advises avoiding bed rest (MedlinePlus, Sciatica), and NICE encourages people to keep up normal activities (NICE NG59, Low back pain and sciatica). Short breaks are fine, but gentle movement through the day tends to serve recovery better than lying still.

Is sciatica the same as a slipped disc?

No. A slipped, or herniated, disc is the most common cause of sciatica, but spinal stenosis and piriformis syndrome can cause it too (MedlinePlus, Sciatica). Sciatica names the leg symptoms; the disc is one possible source.

How is sciatica different from diabetic neuropathy?

Sciatica usually affects one leg and comes from a pressed nerve root. Diabetic neuropathy tends to cause burning or tingling in both feet, often worse at night, and affects up to half of people with diabetes (NIDDK, Peripheral Neuropathy) (MedlinePlus, Sciatica).

Does sciatica come back once it clears up?

It can. MedlinePlus notes that sciatica commonly returns (MedlinePlus, Sciatica), and about 20-30% of people still have problems one or two years after an episode (Koes et al., BMJ 2007). For chronic low back pain, exercise has the strongest support (Hayden et al., Cochrane 2021).
