R-alpha lipoic acid
R-Alpha Lipoic Acid in SciatiEase: Is 250 mg Enough?
R-alpha lipoic acid in SciatiEase comes to 250 mg per 4-capsule serving. Nerve trials used 600 mg a day of standard alpha-lipoic acid, a half R, half S mix that holds about 300 mg of the R form. Benefits showed up mostly with IV doses in diabetic neuropathy. No one has tested it for sciatica.

What it is01
What is R-alpha lipoic acid in SciatiEase?
R-alpha lipoic acid, or R-ALA, is the natural form of alpha-lipoic acid, an antioxidant your body makes in small amounts.
Alpha-lipoic acid comes in two mirror-image forms, R and S. Only the R form is made in the body. Most supplements sold as plain ALA are a racemic mix: half R, half S (Linus Pauling Institute, Lipoic Acid).
SciatiEase uses the R form alone. It sits in the AlphaPalm Blend with PEA, and the label prints its amount: 250 mg per 4 capsules.
The dose02
How does the R-ALA dose in SciatiEase compare with the trials?
The full serving comes fairly close. Two capsules land well short.
The usual trial dose was 600 mg a day of racemic ALA. Half of that, about 300 mg, is the R form. SciatiEase gives 250 mg of R at 4 capsules, roughly 83% of that figure, and 125 mg at 2 capsules (Mijnhout et al., Int J Endocrinol 2012).
That math assumes the R half did the work in those trials. Nobody has proven it, because the human studies did not test pure R (Linus Pauling Institute, Lipoic Acid).

Nerve research03
Does alpha lipoic acid help nerve pain or sciatica?
It eased symptoms in diabetic nerve damage, mostly when given by IV drip. It has not been studied in sciatica.
A meta-analysis pooled trials that scored burning, prickling and numbness on a total symptom scale. ALA beat placebo, with a bigger effect by IV (standardized difference -2.81) than by mouth (-1.78) (Mijnhout et al., Int J Endocrinol 2012).
The authors called IV 600 mg a day for 3 weeks clinically relevant. They were unsure whether the oral gains after 3-5 weeks mattered in daily life. A separate 4-year trial of oral 600 mg missed its main goal (Linus Pauling Institute, Lipoic Acid).
| Finding | Dose and route | Source |
|---|---|---|
| Clear symptom drop | 600 mg a day by IV, 3 weeks | Meta-analysis (Mijnhout et al., Int J Endocrinol 2012) |
| Smaller, less certain drop | Oral, 3-5 weeks | Meta-analysis (Mijnhout et al., Int J Endocrinol 2012) |
| 600 mg as good as 1,200-1,800 mg | Oral, 5 weeks | Review (Linus Pauling Institute, Lipoic Acid) |
| Main goal missed | Oral 600 mg, 4 years | Review (Linus Pauling Institute, Lipoic Acid) |
| SciatiEase | Oral 250 mg R-ALA | Not tested |
R vs standard04
Is R-ALA better than regular alpha lipoic acid?
It may be, but no human trial has shown it. Every study in the main review used the R plus S mix.
Because cells make only the R form, some argue it is the part that counts. It still leaves open whether 250 mg of R matches 600 mg of the mix in people.
About 30-40% of an oral ALA dose is absorbed (Linus Pauling Institute, Lipoic Acid). So the amount that reaches the blood is a fraction of what the label shows, for either form.
With meals05
Should R-alpha lipoic acid be taken with food?
There is a trade-off. Food lowers peak ALA levels by about 30%, but it can settle the stomach.
Nausea, vomiting and diarrhea are among the reported side effects, along with rash and vertigo (Linus Pauling Institute, Lipoic Acid). The sales page pairs the capsules with breakfast and dinner (SciatiEase sales page), which is the gentler choice for most stomachs.
If you skip food, any upset may be easier to notice. The how to take it page covers timing for the whole serving.

Can alpha lipoic acid lower blood sugar?
It may. People on insulin or other diabetes medicine should check their glucose more often after starting it.
A rarer problem is insulin autoimmune syndrome. It is an immune reaction that causes repeated bouts of low blood sugar. A growing number of cases have been linked to ALA supplements, often in people with certain gene types (Vallianou et al., Metabol Open 2026). Stopping the trigger usually leads to a good outcome.
Do not change a diabetes dose on your own because of a supplement. Report shaky, sweaty or confused spells to your doctor. More on supplements and medicines.
The pairing07
Why is R-ALA paired with PEA in SciatiEase?
The brand groups them under one name, AlphaPalm. No study has tested the two together.
Each has its own research base: ALA in diabetic nerve damage, PEA in chronic pain and one sciatica trial. Combining them is a reasonable idea on paper. Single-ingredient results may not hold for a mix, a point the FTC makes about health claims (FTC, Health Products Compliance Guidance).
The brand home page lists 300 mg of R-ALA, while the panel prints 250 mg (SciatiEase order page); we follow the panel.

FAQ09
R-alpha lipoic acid questions
How much alpha lipoic acid is in SciatiEase?
SciatiEase holds 250 mg of R-alpha lipoic acid per 4 capsules, or 125 mg per 2. It is printed inside the 750 mg AlphaPalm Blend, next to 500 mg of PEA. There is no S form on the label.
What is the difference between R-ALA and regular alpha lipoic acid?
Regular ALA is a racemic mix of the R and S forms. R-ALA is the R form alone, the one the body makes. All human studies in the main review used the mix (Linus Pauling Institute, Lipoic Acid).
Has alpha lipoic acid been studied for sciatica?
No trial we found tested it in sciatica. The research is in diabetic nerve damage, where IV doses gave the clearest results and oral benefits were less certain (Mijnhout et al., Int J Endocrinol 2012).
What are the side effects of alpha lipoic acid?
Reported effects include nausea, vomiting, diarrhea, rash and vertigo (Linus Pauling Institute, Lipoic Acid). It may lower blood sugar, and rare cases of insulin autoimmune syndrome with repeated lows have been linked to it (Vallianou et al., Metabol Open 2026).
What alpha lipoic acid dose did nerve trials use?
Most used 600 mg a day of racemic ALA. Over 5 weeks, 600 mg by mouth did as well as 1,200 or 1,800 mg (Linus Pauling Institute, Lipoic Acid). That 600 mg holds about 300 mg of the R form.
Is alpha lipoic acid safe if I have diabetes?
Ask your doctor first. ALA may lower blood sugar, so people on insulin or diabetes pills should test glucose more often (Linus Pauling Institute, Lipoic Acid). Never adjust a diabetes medicine without your prescriber.
