Herniated Disc Sciatica: How a Disc Presses the Nerve

Quick answer

Herniated disc sciatica happens when disc material pushes against a nerve root in the lower spine, sending pain, numbness or tingling down one leg. MedlinePlus names a herniated disk as a leading cause of sciatica (MedlinePlus, Sciatica). Most acute cases improve over time, but about 20 to 30 percent still have problems after one or two years (Koes et al., BMJ 2007).

The cause01

What is herniated disc sciatica?

It is leg pain caused by a bulging or ruptured spinal disc that irritates a nearby nerve root.

Discs are the cushions between spinal bones. When part of one pushes out, it can press on a root that joins the sciatic nerve (MedlinePlus, Sciatica).

The nerve is irritated by pressure and by chemical inflammation around the disc. Piriformis syndrome is another route; see piriformis syndrome vs sciatica.

Recognizing it02

What are the signs of a herniated disc with sciatica?

Typically one leg hurts, often with low back pain that eases or shifts.

Common signs, shown to help you describe symptoms to a clinician
SignWhat people report
PainSharp or burning, from low back or buttock down one leg
Numbness or tinglingAlong the leg or in the foot
WeaknessDifficulty lifting the foot or standing on toes
TriggersSitting, bending, coughing or sneezing

Outlook03

Does a herniated disc heal on its own?

Often the symptoms settle, but the timeline varies.

Most people with acute sciatica have a favorable outlook, and initial conservative care for about 6 to 8 weeks is the consensus approach (Koes et al., BMJ 2007). Disc surgery may relieve leg pain faster than conservative care, yet no clear difference appeared after one or two years (Koes et al., BMJ 2007).

Staying active

Gentle movement is advised over bed rest (MedlinePlus, Sciatica), and NICE suggests considering an exercise program (NICE NG59, Low back pain and sciatica). See sciatica exercises at home.

Do not wait04

When is a herniated disc an emergency?

When it brings loss of bladder or bowel control, saddle numbness or fast-growing leg weakness.

A very large disc herniation can compress the nerves at the base of the spine, called cauda equina syndrome. Surgery within 48 hours of onset gives a significant advantage for recovery (AANS, Cauda Equina Syndrome).

Call a provider for pain over 4 weeks (MedlinePlus, Sciatica). Keep taking prescribed medicine, and talk with a clinician about the next step. Our sciatica relief options guide lists them.

FAQ05

Herniated disc questions

Can a herniated disc cause sciatica without back pain?

Yes. Some people feel the leg pain, numbness or tingling much more than any back ache. A clinician examines the back and leg to find the source.

Do I need an MRI for a herniated disc?

Not always. Many people are treated without imaging at first. A clinician may order one if symptoms are severe, persist or include red flags such as weakness.

Is bed rest good for a slipped disc?

No. MedlinePlus advises avoiding bed rest and staying gently active (MedlinePlus, Sciatica). Short rests are fine, but days in bed tend to stiffen the back.

Is a slipped disc the same as a herniated disc?

People use the terms interchangeably. A disc does not literally slip out of place. Part of it bulges or breaks through its outer layer, which is what the word herniated describes.

Can lifting cause a herniated disc?

Heavy or awkward lifting is a common story, though discs also wear with age. Our sources do not rank causes, so ask your clinician about your own history.
