Ibuprofen for Sciatica: Do NSAIDs Help the Pain?
Ibuprofen for sciatica has weaker evidence than many people expect. A Cochrane review of 10 trials found NSAID pain reduction was not significantly different from placebo in three trials, rated very low quality, and side effects were more common (Rasmussen-Barr et al., Cochrane 2016). Ask your pharmacist or doctor before use, above all with blood thinners.

Does ibuprofen work for sciatica?
The evidence is thin. In pooled trials, NSAIDs did not clearly beat a placebo for sciatica pain.
The Cochrane review included 10 trials with 1,651 people. In three trials covering 918 people, pain fell by 4.56 points on a 100-point scale more than placebo, a gap that was not statistically significant, and the evidence was very low quality (Rasmussen-Barr et al., Cochrane 2016).
NSAIDs did give slightly better global improvement in the review, a low-quality finding, but side effects were more common (Rasmussen-Barr et al., Cochrane 2016).

What do guidelines say about NSAIDs?
They are cautious. NICE asks clinicians to be aware of the harms and limited benefit of NSAIDs in sciatica (NICE NG59, Low back pain and sciatica).
For back pain more broadly, ACP puts non-drug care first, and lists NSAIDs or muscle relaxants when a drug is wanted (Qaseem et al., ACP guideline 2017). Our heat or ice post covers one non-drug option.

Who should ask before taking ibuprofen?
Anyone on a blood thinner, plus anyone with stomach, kidney or heart problems, should check first.
The apixaban (Eliquis) label names aspirin and NSAIDs such as ibuprofen and naproxen as possible interactions (MedlinePlus, Apixaban). Do not stop a prescribed blood thinner on your own; ask your doctor.

Is naproxen better than ibuprofen for sciatica?
We found no head-to-head comparison in our sources. The Cochrane review pooled NSAIDs as a group (Rasmussen-Barr et al., Cochrane 2016). Ask your clinician which, if any, suits your health history.
Can I take ibuprofen with Eliquis?
Ask your doctor first. The apixaban label lists NSAIDs such as ibuprofen as possible interactions and warns of easier bleeding (MedlinePlus, Apixaban). Never stop the blood thinner yourself.
Why do NSAIDs seem to work for my friend's sciatica?
Many cases improve over time regardless of treatment (Koes et al., BMJ 2007), so relief may come from the natural course. Averages in trials do not describe everyone.
Are NSAIDs a first choice for sciatica?
