Acetyl L Carnitine and Chemotherapy Neuropathy: What the Trial Found

Quick answer

Acetyl l carnitine and chemotherapy neuropathy is a cautionary story. In a 409-woman trial on taxane chemotherapy, 3,000 mg a day for 24 weeks made nerve symptoms worse, not better. SciatiEase holds 250 mg per serving, far below that dose, but anyone in cancer treatment should ask their oncologist before taking any supplement.

The trial01

What did the acetyl l carnitine chemotherapy trial find?

It found harm. Women who took ALCAR during chemotherapy ended up with worse neuropathy than women on placebo.

In 409 evaluable women on taxane chemotherapy, 208 took ALCAR at 3,000 mg a day for 24 weeks and 201 took placebo (Hershman et al., J Clin Oncol 2013).

At week 12 there was no difference. By week 24 the ALCAR group scored 1.8 points worse, and grade 3-4 nerve toxicity hit 8 women on ALCAR against 1 on placebo (Hershman et al., J Clin Oncol 2013).

Why one trial matters

A supplement sold for nerves can do the opposite in a setting nobody tested first.

Dose02

How does that dose compare with a SciatiEase serving?

The trial dose was 12 times the amount on the SciatiEase panel.

One 4-capsule serving holds 250 mg of acetyl-L-carnitine. The trial used 3,000 mg, and the diabetic neuropathy studies in the Cochrane review used 1,500 to 3,000 mg (Rolim et al., Cochrane 2019).

Evidence gaps03

Why is the evidence on carnitine for nerve pain so thin?

Because the best review rated it very low certainty.

The Cochrane review of four studies found ALCAR lowered pain by 9.16 mm on a 100 mm scale, with little difference at 1,500 mg or less. Its authors were very uncertain it works (Rolim et al., Cochrane 2019).

What to do04

What should you do if you have chemo neuropathy?

Tell your oncology team about any supplement before you buy it.

Keep taking prescribed medicine. Never stop or change cancer treatment because of a supplement.

  1. Bring the label

Take the Supplement Facts panel to your oncologist or pharmacist.

  2. Report new symptoms

Numb, tingling or burning feet and hands need a call to the care team.

For the wider picture, read ALCAR for nerve pain and the medicines guide.

FAQ05

Carnitine and cancer treatment: specific questions

Can you take acetyl l carnitine during chemotherapy?

Only if your oncologist agrees. The one large trial in women on taxane chemotherapy found 3,000 mg a day made neuropathy worse (Hershman et al., J Clin Oncol 2013).

Does ALCAR prevent chemotherapy neuropathy?

The best trial says no. It tested prevention and found no change at 12 weeks, then significantly worse scores by 24 weeks (Hershman et al., J Clin Oncol 2013).

Did the carnitine chemo trial use SciatiEase?

No. It tested plain acetyl-L-carnitine, 3,000 mg a day, in women with breast cancer (Hershman et al., J Clin Oncol 2013). SciatiEase has not been tested in people having chemotherapy,.

Is 250 mg of acetyl-L-carnitine safe in cancer treatment?

Nobody has tested it. The trial studied only 3,000 mg, so low-dose safety during chemotherapy is unknown. Ask your oncologist.

What are the side effects of acetyl-L-carnitine?

About 3 g a day can cause nausea, vomiting, cramps, diarrhea and a fishy body odor (NIH ODS, Carnitine).
