Acetyl L Carnitine for Nerve Pain: How Strong Is the Evidence?
Acetyl l carnitine for nerve pain has weak support: a Cochrane review of 4 studies and 907 people found about 9 mm less pain on a 100 mm scale, rated very low certainty. Doses were 1,500 to 3,000 mg a day. SciatiEase holds 250 mg per serving, far below those trial doses.

The evidence01
Does acetyl l carnitine help nerve pain?
A little, maybe. The certainty is very low.
Four studies with 907 people (diabetic neuropathy) used 1,500, 2,000 or 3,000 mg a day. Pain fell 9.16 mm on a 100 mm scale versus placebo. At 1,500 mg or less, there was little difference (Rolim et al., Cochrane 2019). Two studies had manufacturer funding (Rolim et al., Cochrane 2019).
Dose gap02
Is 250 mg of acetyl l carnitine enough?
Nothing shows it. It is one sixth of the lowest trial dose.
| Source | Per day |
|---|---|
| Lowest trial dose (Rolim et al., Cochrane 2019) | 1,500 mg |
| SciatiEase, 4 capsules | 250 mg |
| SciatiEase, 2 capsules | 125 mg |
The brand home page lists 300 mg, but the panel says 250 (SciatiEase order page). We follow the panel. Because the dose is so far below the studied range, the trial result cannot be assumed to apply to a SciatiEase serving. It is one of several lines in the formula, not the main one.
Cautions03
What are the risks of acetyl l carnitine?
Stomach upset at high doses, and a harm signal in chemotherapy.
- Around 3 g a day can cause nausea, cramps, diarrhea and a fishy odor (NIH ODS, Carnitine).
- Seizure drugs such as valproic acid lower carnitine (NIH ODS, Carnitine).
- In women on taxane chemotherapy, 3,000 mg worsened neuropathy (Hershman et al., J Clin Oncol 2013).
If you have cancer treatment, ask your oncology team first. See ALCAR and chemotherapy neuropathy.

Is acetyl l carnitine worth it for sciatica?
The evidence is not about sciatica, and the dose in SciatiEase is low.
Read the Supplement Facts panel and benfotiamine for neuropathy. Keep taking prescribed medicine, and never stop it because of a supplement. Carnitine is made by healthy bodies already, so extra is not a need (NIH ODS, Carnitine). Ask a clinician if you take seizure medicine or have cancer treatment ahead.
FAQ05
Acetyl-L-carnitine questions
How many mg of acetyl l carnitine were studied?
1,500, 2,000 or 3,000 mg a day (Rolim et al., Cochrane 2019).
Does ALCAR cause a fishy smell?
At about 3 g a day it can (NIH ODS, Carnitine).
Do healthy people need carnitine pills?
Healthy people make enough (NIH ODS, Carnitine).
Is the Cochrane result strong?
No. It was rated very low certainty (Rolim et al., Cochrane 2019).
Can ALCAR lower seizure drug levels?
Seizure drugs lower carnitine, not the other way (NIH ODS, Carnitine). Ask your pharmacist.
