Benfotiamine for Neuropathy: What the Trial Really Showed

Quick answer

Benfotiamine for neuropathy has modest, mixed support. The main trial gave 165 people with diabetic nerve disease 300 or 600 mg a day for 6 weeks and got a borderline result. SciatiEase holds 200 mg per serving, below both doses tested, and no study looked at it for sciatica.

The trial01

Does benfotiamine work for neuropathy?

Maybe a little. The best trial landed right on the edge of statistical significance.

The BENDIP trial randomized 165 people with diabetic polyneuropathy to 600 mg, 300 mg or placebo for 6 weeks. The Neuropathy Symptom Score improved in the per-protocol group (p = 0.033) but just missed in the intention-to-treat group (p = 0.055) (Stracke et al., BENDIP 2008).

The Total Symptom Score showed no significant difference. Effects were bigger at the higher dose, and people tolerated the capsules well (Stracke et al., BENDIP 2008).

One analysis passed and the other narrowly failed. That is a hint worth more study, not proof.

Wider evidence02

What do reviews say about benfotiamine and B vitamins?

They say the evidence is small and not settled.

A Cochrane review of 13 studies with 741 people found vitamin B evidence insufficient to say if it helps or harms. One small trial saw slightly better vibration sense with oral benfotiamine over 8 weeks (Ang et al., Cochrane 2008).

Another overview lists three small studies using 120 to 900 mg that lowered symptom severity against placebo (NIH ODS, Thiamin). All of this work was in diabetic neuropathy, not sciatica.

Dose03

How much benfotiamine is in SciatiEase?

A 4-capsule serving holds 200 mg, and 2 capsules hold 100 mg.

The brand home page lists 300 mg (SciatiEase order page), but the panel prints 200 mg, and this site follows the panel.

So the label sits under every dose in the trial. That does not make it useless, but it means the trial result cannot be assumed to carry over.

Next step04

Who should talk to a clinician first?

Anyone with numb or burning feet, because the cause matters more than the capsule.

Diabetic neuropathy affects up to half of people with diabetes (NIDDK, Peripheral Neuropathy). A clinician can check blood sugar and B12. Keep taking prescribed medicine, and never change it because of a supplement.

FAQ05

Benfotiamine and nerve symptoms: specific questions

What dose of benfotiamine was used in neuropathy trials?

The BENDIP trial used 300 and 600 mg a day for 6 weeks (Stracke et al., BENDIP 2008). Other small studies ranged from 120 to 900 mg (NIH ODS, Thiamin). SciatiEase's 200 mg per serving is below that range.

How long does benfotiamine take to work for nerves?

The main trial measured symptoms after 6 weeks (Stracke et al., BENDIP 2008). A Cochrane trial looked at 8 weeks (Ang et al., Cochrane 2008). Neither shows when, or whether, a given person will notice change.

Is benfotiamine proven for sciatica?

No. The trials we found enrolled people with diabetic neuropathy, not sciatica. Sciatica usually comes from a pressed nerve root (MedlinePlus, Sciatica), a different cause.

Does benfotiamine have side effects?

The BENDIP trial reported the capsules were well tolerated (Stracke et al., BENDIP 2008). Cochrane also called vitamin B generally well tolerated (Ang et al., Cochrane 2008). Long-term data are limited.

Is benfotiamine just another name for thiamine?

Not exactly. It is a synthetic thiamin derivative that the body converts to thiamin (NIH ODS, Thiamin). Our benfotiamine vs thiamine post compares them further.
