Do Neuropathy Supplements Work? The Review Evidence

Quick answer

Do neuropathy supplements work? Sometimes a little, and the certainty is low. A Cochrane review could not say whether B vitamins help or harm. Acetyl-L-carnitine trimmed pain by about 9 mm on a 100 mm scale, rated very low certainty. Alpha-lipoic acid looks best when given by IV, not by mouth.

The verdict01

Do neuropathy supplements work, according to reviews?

The honest summary is possible small benefits, weak proof and a few risks.

Most trials are in diabetic neuropathy, where up to half of people with diabetes have nerve symptoms (NIDDK, Peripheral Neuropathy). Supplements are one small piece. Blood sugar control, blood pressure and cholesterol matter more for protecting nerves (NIDDK, Peripheral Neuropathy).

Below is what the main reviews and trials say, ingredient by ingredient.

The numbers02

What do trials say about each neuropathy supplement?

Each ingredient has a different size and quality of evidence.

Reviews of common neuropathy supplement ingredients
IngredientMain finding
B vitamins13 studies, 741 people: evidence insufficient (Ang et al., Cochrane 2008)
Alpha-lipoic acidIV 600 mg for 3 weeks helped; oral relevance unclear (Mijnhout et al., Int J Endocrinol 2012)
Acetyl-L-carnitinePain down 9.16 mm, very low certainty (Rolim et al., Cochrane 2019)
BenfotiamineBorderline at 300 and 600 mg a day, 6 weeks (Stracke et al., BENDIP 2008)

Oral lipoic acid at 600 mg a day matched higher oral doses in a 5-week comparison (Linus Pauling Institute, Lipoic Acid), but that is not the same as beating placebo.

Cautions03

Can neuropathy supplements cause harm?

Yes. The same nerves you want to protect can be hurt by the wrong dose.

Two documented cases

Acetyl-L-carnitine at 3,000 mg a day worsened chemotherapy neuropathy in a 409-woman trial (Hershman et al., J Clin Oncol 2013). High vitamin B6 intake can itself cause neuropathy (NIH ODS, Vitamin B6).

Lipoic acid may lower blood sugar, which matters with diabetes medicine (Linus Pauling Institute, Lipoic Acid).

Choosing04

How do you choose a neuropathy supplement wisely?

Match the dose on the panel to the dose in the trials, and bring your clinician in.

Read each ingredient's amount against its studied range. Our nerve supplements compared page does this for SciatiEase, and the lipoic acid dosage and benfotiamine for neuropathy posts cover two lines in detail. The ingredients page lists all 12.

FAQ05

Neuropathy supplements: sharper questions

Which neuropathy supplement has the strongest evidence?

Intravenous alpha-lipoic acid at 600 mg a day for 3 weeks has the clearest results, in diabetic neuropathy (Mijnhout et al., Int J Endocrinol 2012). Oral supplements are less certain, and none of this applies directly to a mixed formula.

Do neuropathy supplements work for non-diabetic nerve pain?

Data are thinner. Most trials enrolled diabetic or chemotherapy patients (Rolim et al., Cochrane 2019) (Ang et al., Cochrane 2008). Results from those groups may not carry over to other causes.

How long should I try a neuropathy supplement?

Trials ran from about 3 weeks to a few months (Mijnhout et al., Int J Endocrinol 2012) (Stracke et al., BENDIP 2008). If nothing has changed by then, talk with your clinician instead of raising the dose.

Can supplements slow neuropathy from getting worse?

No trial we cite shows that. The steps with support are blood sugar, blood pressure and cholesterol control (NIDDK, Peripheral Neuropathy). Supplements target symptoms, not the disease course.

Is a high-dose B12 enough for neuropathy?

Only if B12 is low. A 1,000 mcg oral dose is absorbed at about 1.3% (NIH ODS, Vitamin B12), and metformin or acid reducers can lower B12 levels. A blood test tells you whether it applies to you.
