Nerve Supplements for Neuropathy: Evidence by Ingredient

Quick answer

Nerve supplements for neuropathy have mixed evidence, mostly from diabetic neuropathy. Alpha-lipoic acid has the clearest result, but mainly as 600 mg intravenously for 3 weeks. Carnitine is very low certainty, benfotiamine borderline, and B vitamins inconclusive. High-dose carnitine worsened chemotherapy neuropathy, so ask your oncologist before using any.

Overview01

Do nerve supplements help neuropathy?

Some may ease symptoms a little, and none is shown to restore nerves.

Up to half of people with diabetes have peripheral neuropathy, with burning, tingling and numbness in the feet and legs (NIDDK, Peripheral Neuropathy). Managing blood glucose, blood pressure and cholesterol can keep damage from worsening (NIDDK, Peripheral Neuropathy).

Prescribed pain drugs relieve pain but do not reverse nerve damage (NIDDK, Peripheral Neuropathy). Ask a doctor before adding supplements.

Ingredient by ingredient02

What does the evidence say for each ingredient?

Each has a small, specific result, and none has a large one.

Alpha-lipoic acidIV 600 mg for 3 weeks clearest; oral unclear (Mijnhout et al., Int J Endocrinol 2012)R-ALA 250 mg

Trials used racemic alpha-lipoic acid, not R-ALA alone (Linus Pauling Institute, Lipoic Acid).

Safety03

Can a nerve supplement make neuropathy worse?

Yes, in one trial, so it is a real question.

Chemotherapy neuropathy

In 409 women on taxane chemotherapy, acetyl-L-carnitine at 3,000 mg a day significantly worsened neuropathy by 24 weeks (Hershman et al., J Clin Oncol 2013). See carnitine and chemotherapy neuropathy.

High B6 can also injure nerves, and symptoms have been reported below 50 mg a day (TGA, Vitamin B6 and neuropathy). Keep taking prescribed medicine and ask first if you have chemotherapy.

Should you test for B12 before trying supplements?

If you take metformin or an acid reducer, ask about it.

Metformin and acid reducers can lower B12, and deficiency causes numbness and tingling that can become permanent if missed (NIH ODS, Vitamin B12). Compare options on the supplements compared page.

FAQ05

Neuropathy supplements: specific questions

Is alpha-lipoic acid good for neuropathy?

It has the best result, mainly with 600 mg given intravenously for 3 weeks. Whether oral doses help is unclear (Mijnhout et al., Int J Endocrinol 2012).

Does benfotiamine help diabetic neuropathy?

One 6-week trial found a borderline symptom benefit at 300 or 600 mg a day (Stracke et al., BENDIP 2008). Evidence is small, and one score showed no difference.

Which B vitamin is best for neuropathy?

None is proven. A Cochrane review found the evidence insufficient to judge benefit or harm (Ang et al., Cochrane 2008). Correcting a real B12 deficiency is the clear case (NIH ODS, Vitamin B12).

Can nerve supplements replace diabetes medicine?

No. Blood glucose control is what protects nerves, and supplements are an addition at most (NIDDK, Peripheral Neuropathy). Do not change prescribed medicine without your clinician.

Does alpha-lipoic acid affect blood sugar?

It may lower it, so people on insulin or diabetes drugs should monitor glucose (Linus Pauling Institute, Lipoic Acid). Rare cases of recurrent low blood sugar have been reported (Vallianou et al., Metabol Open 2026).
