Sciatica Surgery: When It Is Considered and Evidence Basics

Quick answer

Sciatica surgery, often a disk operation, is usually considered only after other treatments fail, except in emergencies such as cauda equina syndrome (NIAMS, Living With Back Pain) (AANS, Cauda Equina Syndrome). A BMJ review found disk surgery may relieve leg pain faster than conservative care, with no clear difference after one or two years (Koes et al., BMJ 2007). Your surgeon should explain the options for your case.

When01

When is sciatica surgery considered?

Usually when other treatments have not worked, and urgently when cauda equina signs appear.

NIAMS says surgery for back pain is for when other treatments fail (NIAMS, Living With Back Pain). The BMJ review notes initial conservative treatment for about 6 to 8 weeks is the consensus approach (Koes et al., BMJ 2007).

The exception is an emergency. Urinary retention, incontinence, saddle numbness and leg weakness need immediate attention, and surgery within 48 hours gives a significant advantage (AANS, Cauda Equina Syndrome).

Evidence02

Does sciatica surgery work better than waiting?

It may bring faster leg pain relief, but results looked similar after one or two years.

TimeWhat the review found
EarlyDisk surgery may relieve leg pain faster
After 1 to 2 yearsNo clear difference found
Overall outlookMost acute sciatica improves; 20% to 30% have problems after 1-2 years

All of this is from the 2007 review (Koes et al., BMJ 2007), so ask your surgeon for current figures. Our herniated disc post covers the usual cause.

Before surgery03

What should you tell your surgeon about supplements?

Tell them everything you take, well before the date.

Bleeding risk

NCCIH says to tell providers about all supplements well before surgery, since some raise bleeding risk (NCCIH, Using Dietary Supplements Wisely). Feverfew and Chinese skullcap may affect clotting (MSKCC, Feverfew) (MSKCC, Scutellaria baicalensis).

Do not stop a prescription on your own; the surgical team will give instructions. See our supplement and surgery post.

Questions04

What questions should you ask a surgeon?

Ask what the operation aims to fix and what the alternatives are.

  • What exactly is pressing on my nerve?
  • What are the risks and the recovery time?
  • What happens if I wait or try other care first? See relief options.

FAQ05

Sciatica surgery questions

What is the most common surgery for sciatica?

Operations on a herniated disk are the usual type; the BMJ review discusses disk surgery (Koes et al., BMJ 2007). Your surgeon decides the procedure from your scans and exam.

How fast does surgery relieve sciatica pain?

The review says disk surgery may relieve leg pain faster than conservative care (Koes et al., BMJ 2007). Individual recovery varies. Ask your surgeon what to expect for your procedure.

Is sciatica surgery ever an emergency?

Yes, for cauda equina syndrome. Red flags include urinary retention, incontinence and saddle numbness; surgery within 48 hours gives a significant advantage (AANS, Cauda Equina Syndrome). Seek care at once.

Should I stop my supplements before surgery?

Ask your surgeon; do not guess. NCCIH advises telling providers well before surgery because some supplements raise bleeding risk (NCCIH, Using Dietary Supplements Wisely). Follow their written instructions.

Can surgery be avoided?

Often, since most acute sciatica improves with time (Koes et al., BMJ 2007). Surgery is typically considered after other treatments fail (NIAMS, Living With Back Pain). Your clinician can say how your case compares.
