Nerve Supplements for Sciatica: Which Have Any Evidence?

Quick answer

Nerve supplements for sciatica have thin evidence. Only palmitoylethanolamide (PEA) has a sciatica trial we can cite: 636 patients took 300 or 600 mg a day for 3 weeks. Other ingredients were studied in diabetic neuropathy, not sciatica. Most sciatica improves on its own, and exercise has better support than any supplement.

Cause first01

Can nerve supplements help sciatica?

Possibly a little, but only one ingredient has sciatica data, and it is limited.

Sciatica is a symptom, usually of a herniated disk, spinal stenosis or piriformis syndrome pressing on the nerve (MedlinePlus, Sciatica). A supplement does not move the disk. It often gets better on its own, though it commonly returns (MedlinePlus, Sciatica), and about 20% to 30% still have problems after 1 to 2 years (Koes et al., BMJ 2007).

That is the frame for every ingredient below.

Ingredient by ingredient02

Which ingredients have sciatica evidence?

PEA is the only one with a sciatica trial.

Evidence by ingredient, sciatica versus other nerve conditions
IngredientStudied in
PEASciatic pain: 636 patients, 3 weeks, 300 or 600 mg (Keppel Hesselink, J Pain Res 2015)
Alpha-lipoic acidDiabetic neuropathy (Mijnhout et al., Int J Endocrinol 2012)
Acetyl-L-carnitineDiabetic neuropathy (Rolim et al., Cochrane 2019)
BenfotiamineDiabetic neuropathy (Stracke et al., BENDIP 2008)
B vitaminsAlcoholic and diabetic neuropathy (Ang et al., Cochrane 2008)

The PEA trial appeared only in a Spanish journal, and the NNT figures came from a post hoc analysis (Keppel Hesselink, J Pain Res 2015). Read PEA for sciatica for the detail.

What helps first03

What does guidance recommend before supplements?

Staying active and gentle exercise come first.

NICE recommends self-management advice, normal activity and a group exercise program, and advises against gabapentinoids and oral steroids for sciatica (NICE NG59, Low back pain and sciatica). Pregabalin did no better than placebo in a trial of 209 people (Mathieson et al., NEJM 2017 (PRECISE)). Never stop a prescribed medicine on your own; talk with your prescriber.

Get urgent care

Loss of bladder or bowel control, numbness around the groin or buttocks, or leg weakness needs care right away (AANS, Cauda Equina Syndrome). Call a provider if pain lasts over 4 weeks (MedlinePlus, Sciatica).

Where does SciatiEase fit?

As an optional extra with PEA at 500 mg per serving, between the trial's two doses.

The other lines have no sciatica evidence. See sciatica relief options for exercise and care, and nerve supplements for neuropathy for the wider evidence.

FAQ05

Sciatica and supplements: specific questions

Is PEA good for sciatica?

It has the only sciatica trial we can cite: 636 patients, 3 weeks, 300 or 600 mg a day, with better results at 600 mg (Keppel Hesselink, J Pain Res 2015). The trial has limits, so results are not certain.

Can B12 help a sciatic nerve?

B12 matters if you are deficient, since deficiency can cause numbness and tingling (NIH ODS, Vitamin B12). We found no trial of B12 for sciatica in people with normal levels.

Do supplements work as well as ibuprofen for sciatica?

No head-to-head trial exists. NSAIDs were not significantly better than placebo for sciatica pain in the trials reviewed (Rasmussen-Barr et al., Cochrane 2016). Ask your doctor about your own medicines.

How long should I try a supplement for sciatica?

The PEA trial lasted 3 weeks (Keppel Hesselink, J Pain Res 2015). Track your pain at day 30 and see a provider if it persists past 4 weeks (MedlinePlus, Sciatica).

Does exercise beat supplements for sciatica?

On evidence, yes. Exercise reduces chronic back pain with moderate certainty (Hayden et al., Cochrane 2021), and NICE recommends it (NICE NG59, Low back pain and sciatica).
